By checking the acknowledgement box on the registration form, the parent or guardian confirms they have read, understood, and agree to the terms set out below on behalf of themselves and their child.


ABOUT THIS DOCUMENT

This document is an Assumption of Risk and Parental Consent form. It is intended to inform you of the physical risks associated with participating in Union Elite Basketball programs, camps, tryouts, and training sessions, and to obtain your informed consent for your child's participation.


Union Elite Basketball Academy Inc. strongly recommends that all parents and guardians read this document carefully and in full before completing registration. If you have questions about any of the terms below, please contact us at info@unionelitebasketball.com before proceeding.


NATURE OF THE ACTIVITY

Basketball is a physically demanding contact sport. Participation in Union Elite Basketball programs, camps, tryouts, and training sessions involves physical exertion, physical contact with other players, and the use of sports equipment and facilities.


Physical risks associated with participation include but are not limited to:


  • Sprains, strains, and muscle injuries resulting from running, jumping, pivoting, and physical contact.
  • Fractures and broken bones resulting from falls, collisions, or physical contact with other players.
  • Head and neck injuries, including concussion, resulting from contact with other players, the floor, walls, or equipment.
  • Eye and dental injuries resulting from contact with the ball, other players, or equipment.
  • Cardiovascular stress resulting from intense physical activity.
  • Cuts, bruises, and abrasions resulting from contact with the court surface, equipment, or other players.
  • Injuries resulting from the actions of other participants that are beyond the reasonable control of Union Elite Basketball and its staff.
  • Illness resulting from shared use of facilities and equipment.
  • In rare and extreme circumstances, death.


These risks exist even when Union Elite Basketball and its coaches take all reasonable precautions. No sports environment can be made entirely free of risk.


ACKNOWLEDGEMENT OF RISK


By completing registration, the parent or guardian acknowledges and accepts that:

  • They have read and understood the risks described above.
  • Their child's participation in Union Elite Basketball activities is voluntary.
  • They are aware that injuries can and do occur in basketball despite reasonable precautions being taken by the organization and its staff.
  • They have had the opportunity to ask questions and seek clarification before agreeing to these terms.


CONDUCT AND SAFETY

Union Elite Basketball is committed to providing a safe training environment. Coaches and staff are trained to manage physical activity safely and to respond to injuries and medical emergencies. However, participants are also responsible for following all safety instructions provided by coaching staff, playing within the rules of the activity, and reporting any injuries or medical concerns promptly.


Union Elite Basketball reserves the right to remove any participant from a program whose conduct is determined to pose a risk to themselves or others.


PARENTAL CONSENT FOR EMERGENCY MEDICAL TREATMENT

In the event of illness or injury during any Union Elite Basketball activity, I give permission for emergency medical treatment to be sought for my child by qualified medical personnel. I authorize the person in charge to take my child to the nearest hospital or certified medical facility. I give consent for that facility to provide any and all necessary emergency medical care.


Union Elite Basketball will make every reasonable effort to contact the parent or guardian before seeking emergency treatment. Where this is not possible due to the urgency of the situation, emergency treatment will be sought without delay.


INDEMNIFICATION

To the fullest extent permitted by law, the parent or guardian agrees to indemnify and hold harmless Union Elite Basketball Academy Inc., its directors, officers, coaches, managers, administrative staff, volunteers, and facility partners from any and all claims, demands, damages, costs, and expenses arising from or connected to their child's participation in Union Elite Basketball activities, where such claims arise from risks inherent to the sport of basketball as described in this document.


This indemnification does not extend to claims arising from gross negligence or willful misconduct on the part of Union Elite Basketball or its staff.


GOVERNING LAW

This document is governed by the laws of the Province of Ontario and the applicable federal laws of Canada.
